Preoperative Computed Tomographic Angiogram for Deep Inferior Epigastric Artery Perforator Flap for Breast Reconstruction: The Imaging Mapping Era

چکیده
Surgical techniques for breast reconstruction have been greatly refined in last decades and important landmarks have been achieved. Autologous reconstruction with microsurgical flaps marked a before and after in breast reconstruction because the aesthetic outcome is improved, a prosthesis is not needed and long-term results are good. Different types of flaps have been used to create a new breast with similar characteristics in shape, size, contour and position to the contralateral one. The preferred donor site for this purpose is the abdominal wall. The deep inferior epigastric artery perforator flap (DIEP) provides fat and skin with characteristics that are very similar to those of the normal breast and spares the rectus abdominis muscle or fascia, thereby minimizing donor site morbidity (1-3). In function of these advantages the DIEP flap has become the gold standard for breast reconstruction in many hospitals (4).

The Utility of Computed Tomographic Angiography in Preoperative Planning for Deep Inferior Epigastric Artery Flap Breast Reconstruction
Introduction: Abdominal donor site flaps are the standard for autologous breast reconstruction. Because of extreme variations of abdominal wall vascular anatomy between individuals, and even two hemi-abdomens of the same person, it seems valuable to use a sensitive method for preoperative evaluation. In this study, we evaluated the effects of preoperative abdominal wall computed tomographic ang...
متن کاملA Preoperative Marking Template for Deep Inferior Epigastric Artery Perforator Flap Perforators in Breast Reconstruction
Preoperative perforator marking for deep inferior epigastric artery perforator flaps is vital to the success of the procedure in breast reconstruction. Advances in imaging have facilitated accurate identification and preselection of potentially useful perforators. However, the reported imaging accuracy may be lost when preoperatively marking the patient, due to 'mapping errors', as this relies ...
متن کاملCASE REPORT Case Report and Review of the Literature: Deep Inferior Epigastric Perforator Flap for Breast Reconstruction After Abdominal Recontouring
OBJECTIVE The report herein presents a case of a 49-year-old woman with left breast cancer who presented seeking immediate autologous reconstruction. Surgical history included an abdominal hysterectomy and an abdominal contouring procedure. This is a first description of a deep inferior epigastric perforator flap after abdominal wall manipulation of this magnitude. METHODS Computed tomographi...
متن کاملAutologous breast reconstruction: preoperative magnetic resonance angiography for perforator flap vessel mapping.
BACKGROUND Selection of a vascular pedicle for autologous breast reconstruction is time consuming and depends on visual evaluation during the surgery. Preoperative imaging of donor site for mapping the perforator artery anatomy greatly improves the efficiency of perforator selection and significantly reduces the operative time. In this article, we present our experience with magnetic resonance ...
متن کاملEnhanced Preoperative Deep Inferior Epigastric Artery Perforator Flap Planning with a 3D-Printed Perforasome Template: Technique and Case Report
Optimizing preoperative planning is widely sought in deep inferior epigastric artery perforator (DIEP) flap surgery. One reason for this is that rates of fat necrosis remain relatively high (up to 35%), and that adjusting flap design by an improved understanding of individual perforasomes and perfusion characteristics may be useful in reducing the risk of fat necrosis. Imaging techniques have s...
